that all NDIS consultations with Tailored Dietetics will be delivered via their secure Online Clinic. I agree to complete an intake form (which takes around 10–15 minutes) and sign a separate Service Agreement outlining the conditions of NDIS services. To support my initial consultation, I understand I’m encouraged to complete a 3-day food diary, either written or by keeping a photo log, and send it through prior to my appointment along with any relevant blood test where applicable.
